If your child's insurance card says Cigna, ABA benefits are usually reviewed by Evernorth Behavioral Health, part of the same company. Evernorth publishes the rules its reviewers use in a coverage policy called EN0499, "Intensive Behavioral Interventions." This guide walks through what that policy asks for, how Florida law fits in, and a checklist you can use with your provider. It is based on the version effective May 15, 2026, with a next review date of December 15, 2026.
Does Cigna cover ABA therapy?
Many Cigna plans cover ABA for autism, but coverage depends on your plan. EN0499 itself says: "In the absence of a controlling federal or state coverage mandate, benefits are ultimately determined by the terms of the applicable benefit plan document." It also says a customer's plan document "always supersedes the information in the Coverage Policies" if they conflict.
In Florida that leads to two situations:
| Your Cigna plan | What controls | What it means |
|---|---|---|
| Fully insured Florida large-group plan or HMO | Florida's Geller Act plus your certificate and EN0499 | ABA for autism and Down syndrome must be covered for eligible children; EN0499 guides medical necessity review |
| Self-funded employer plan administered by Cigna | Your employer's plan document plus EN0499 | ABA is covered only if the plan includes it; the Florida mandate does not apply |
| Individual or small-employer plan | The plan's design | ABA may or may not be covered |
A plan can be self-funded by your employer even when the card shows Cigna. Ask HR whether your plan is fully insured or self-funded. See fully insured vs self-funded plans.
What does Cigna need before ABA can start?
A confirmed autism diagnosis
EN0499 requires a confirmed diagnosis of autism spectrum disorder (ICD-10 codes F84.0 to F84.9, "with the exception of F84.2, Rett syndrome") based on DSM-5-TR criteria, made by a professional licensed to practice independently whose licensing board considers diagnosis within their scope. The request must list the diagnosing professional's name, credentials and type of license, and "The date on which the diagnosis was most recently made."
A standardized assessment, done recently
The ABA assessment must be done by a Board Certified Behavior Analyst (BCBA), a behavior analyst holding a state license where one exists (Florida has no such license), or an independently licensed mental health clinician with documented ABA training. It must use a reliable, valid and standardized instrument, completed in full, in its current version: the policy's example is "must be the Vineland-3 vs. Vineland-II." For initial treatment, the assessment must be completed "within 60 days prior to the start of treatment," and baseline data must also be collected within 60 days before treatment starts.
A detailed treatment plan
- Clearly defined, measurable goals for each setting where treatment happens (home, clinic, school, community), tied to the assessment and to the core features of autism.
- Dated baseline data and mastery criteria for each goal, with data reported separately for each setting.
- Treatment intensity that "reflects the severity of the impairments, goals of treatment, and/or response to treatment."
- Supervision by a BCBA or qualified licensed clinician "consistent with the general accepted standard of care of one to two hours per ten hours of direct treatment," and at least one to two hours a week when direct treatment is 10 hours a week or less.
- Stakeholder (parent or caregiver) training with measurable goals, mastery criteria and data.
- A plan for generalization, a "transition plan that includes a plan for fading services," and individualized discharge criteria.
- Coordination with other providers and with school services.
For what these parts look like in practice, see inside an ABA treatment plan.
Does Cigna require prior authorization for ABA?
Yes for treatment. Evernorth's ABA prior authorization form (PCOMM-2025-224) says Evernorth "requires prior authorization for the applied behavior analysis (ABA) level of care, including network exception requests." It also says that for standard requests, the assessment codes 97151, 97152 and 0362T "no longer require prior authorization." A network exception request, for an out-of-network assessment, still needs the form.
According to the same form, approved treatment is authorized on a monthly basis, and "ABA hours requested per week are multiplied by 4.33 to determine monthly averages." Your provider usually submits the request. For the general process, see ABA prior authorization in Florida.
What does Cigna need to keep approving ABA?
Continued treatment must still meet the starting criteria, plus:
- an updated treatment plan;
- current data collected within 60 days before the request, showing "ongoing and sustained progress";
- a standardized assessment completed no more than one year before the continued treatment request;
- a new standardized assessment after "any break in treatment greater than 60 calendar days";
- data showing parents and other stakeholders "continue to actively participate in the treatment"; and
- if progress is slow, a documented plan to address barriers and adjust the program.
Requests to increase hours need a clinical rationale and data showing how more hours would help. See avoiding a gap at reauthorization.
What does Cigna not cover?
- ABA "for all non-autism spectrum disorders (ASD) indications including Rett syndrome." In a fully insured Florida large-group plan, Florida law separately requires coverage of ABA for Down syndrome; see ABA for Down syndrome coverage.
- "Services that are considered primarily educational or vocational in nature, or related to academic or work performance."
- ABA delivered to the same person "at the same time as any other treatment modality (e.g., ABA and speech therapy, or ABA and occupational therapy)."
- ABA used to replace a classroom aide, 1:1 teacher, tutor, vocational coach or respite.
- Intensive behavioral interventions other than ABA.
Services from more than one ABA agency in the same authorization period are covered only when the plan is coordinated, consistent and not duplicative.
How does Cigna's policy line up with Florida law?
For a fully insured Florida plan, both the statute and EN0499 apply. They mostly fit together, with one important exception.
| Topic | Florida Geller Act (fully insured plans) | Evernorth EN0499 |
|---|---|---|
| Conditions | Autism and Down syndrome | Autism only; other conditions not medically necessary |
| Age | Under 18, or 18+ in high school if diagnosed by 8 | No age limit stated; plan terms apply |
| Paperwork | Physician-prescribed treatment plan with goals, frequency, update schedule and physician signature | Detailed diagnosis, assessment, goals, data and supervision requirements |
| Review | Plans may use medical necessity review and network rules | Prior authorization for treatment, continued review with data |
| Habilitative care | Cannot be denied for being habilitative | Not addressed as a reason for denial |
Where they conflict, the policy itself defers to "a controlling federal or state coverage mandate." For the full statute, see Florida's autism insurance law explained.
Cigna ABA checklist for Florida parents
- Ask HR whether your Cigna plan is fully insured or self-funded.
- Call the behavioral health number on the card: ask whether ABA is covered, your deductible, copay or coinsurance, and your out-of-pocket maximum.
- Find the diagnostic report and check it lists the evaluator's name, credentials, license and date.
- Schedule the ABA assessment close to the planned start date, because of the 60-day window.
- Plan for your own participation in parent training; Cigna looks for data on it.
- Mark the next reauthorization date and avoid breaks longer than 60 days when possible.
- If ABA is denied, get the denial in writing and see how to appeal an ABA denial.
How Hope Center works with Cigna members
Hope Center for Behavior Change works with Cigna (Evernorth). Our BCBAs complete the assessment and write the treatment plan, the clinical director reviews every plan before it goes to insurance, and parent training happens weekly, which supports the participation data Cigna reviews. Approved hours and your costs depend on your plan, so we start with a free benefits check. See insurance at Hope Center, request a free benefits check, or call 561-337-8865.
